How To Configure A Symptoms Screening Form

1. Log in to the Athena Admin Control Center with Username and Password provided by Athena Support or your organization's admin. 


Login at http://temp.athena-security.com


Once logged in, the Alerts view is the default view. In the top left, you will see your organization's name instead of "iosapp" is the name of our demonstration environment below.

2. Click on Screening Forms



3. Click on New





3. Fill in the information:

    (1.) Symptom Screening Title for your audience

    (2.) Box- location this belongs 

    (3.) Question Text note you can put HTML in this field for spacing requirements

    (4.) Save
